Awake breast surgery
Awake Breast Augmentation Over the Muscle in Ottawa
In over-the-muscle (prepectoral or subfascial) breast augmentation the implant sits in front of the chest muscle instead of behind it. For selected patients it can be performed awake, with local anesthesia and light sedation.

What is breast augmentation over the muscle?
The implant is placed through an incision into a pocket above the pectoralis muscle, often beneath a thin layer of tissue called fascia. Because the chest muscle is not lifted, the operation may involve less muscle-related soreness, and the implant does not move when the muscle flexes.
Who may be a good candidate
- Adults who want more breast volume and have enough breast tissue and skin thickness to cover an implant
- People who want to avoid muscle-related soreness or implant movement with chest exercise
- Patients in good general health who would prefer to avoid a general anesthetic, if suitable
- Less ideal if tissue is very thin, where under-the-muscle placement or fat transfer may look more natural
Why awake breast augmentation over the muscle may work
Published series describe breast augmentation under local anesthesia with light sedation, including submuscular augmentation with tumescent local anesthesia, intercostal nerve blocks, and an endoscopic approach. Placing the implant over the muscle avoids dissecting the muscle, which is the part of augmentation that tends to cause the most discomfort, so an awake plan is often a good fit. Being awake also lets you sit up so symmetry can be checked as we work.
When awake may not be the right choice
An awake approach may not be advised if you are very anxious, have pain that is hard to control, need extensive additional work, or have health conditions that make sedation less safe. In those cases we arrange general anesthesia.
Your awake breast augmentation over the muscle journey
1. Before surgery (pre-op)
Implant size, shape and incision are planned at consultation. Sizers and visual simulation help set expectations but cannot guarantee a result.
- A consultation to review your goals, health history, medications and previous surgeries, and a physical examination
- An anesthesia assessment to decide whether an awake plan is appropriate and safe for you
- Photographs, with your consent, and a written plan, quote and consent form
- Medication guidance, including when to stop blood thinners, certain supplements, and smoking or vaping
- Fasting instructions from the anesthesia team. Follow them exactly
- A ride home and an adult to stay with you after surgery
2. Day of surgery (intra-op)
After your markings, local anesthetic is infiltrated around the breast pockets and a measured amount of sedation may be added. Implants are placed, and you may be asked to sit up so symmetry can be checked.
- You arrive, change, and meet the surgeon and anesthesiologist, and your markings and plan are confirmed
- An IV and monitors are placed. The anesthesiologist stays with you from start to finish
- Local anesthetic is given. Expect brief stinging, then numbness, and light sedation if planned
- You can listen to music, talk with the team, and tell us at any time if you feel discomfort
- Afterward you rest in recovery, receive written instructions and go home with your driver
3. Recovery (post-op)
You are observed in recovery, given written instructions and pain-control guidance, and scheduled for follow-up. Call us or seek urgent care for fever, spreading redness, severe or worsening pain, heavy bleeding, chest pain, shortness of breath, or leg pain or swelling.
| When | What is typical |
|---|---|
| Days 1 to 3 | Tightness and soreness, usually managed with simple pain medication as directed. A supportive bra is worn. |
| Week 1 | Many people return to desk work within a few days to a week. No heavy lifting. |
| Weeks 2 to 6 | Exercise is resumed gradually as advised. Swelling settles and implants soften. |
| 3 to 6 months | Implants settle into position and the result matures. |
Timelines are general and vary. Your surgeon gives you instructions specific to you.
Anesthesia options
Anesthesia options range from local anesthesia, with or without sedation, to general anesthesia. An anesthesiologist is present throughout your procedure and recommends an approach after reviewing your health, the extent of surgery and your preferences. If an awake approach is not suitable, general anesthesia is arranged. Plans can change during surgery if comfort or safety requires it, including deepening sedation or converting to general anesthesia.

Alternatives
Under-the-muscle implants, fat transfer for modest volume, a breast lift if the main concern is position, or no surgery.
Risks
Bleeding or hematoma, infection, capsular contracture, implant rippling or visibility (more likely with thin tissue), malposition, asymmetry, changes in nipple sensation, scarring, implant rupture, the need for future surgery, rare implant-associated conditions, and anesthesia-related risks.
Anesthetic risks include reactions to medications, breathing or airway events, the need to deepen sedation or convert to general anesthesia, and, rarely, transfer for further care. All surgery carries risk, and results are not guaranteed.
Frequently asked questions about awake breast augmentation over the muscle
What is the difference between over and under the muscle?
Over-the-muscle places the implant in front of the pectoralis muscle. Under-the-muscle places it partly behind. The choice depends on your tissue thickness, goals and lifestyle.
Can I really be awake for breast augmentation?
In selected patients, yes. Published series describe augmentation under local anesthesia with light sedation. We confirm suitability at consultation and can switch to general anesthesia if needed.
Will I feel pain?
The breast is numbed, but you may feel pressure or tugging. Sedation can be adjusted and you can tell us if anything is uncomfortable.
Does over-the-muscle placement cause more rippling?
It can in people with thin tissue, which is why we assess your tissue thickness before recommending this plane.
How long until I can exercise?
Usually a gradual return over several weeks. You receive a specific plan after surgery.
