The 45-minute wait: why numbing takes patience
In one breast series, surgeons waited 45 minutes after infiltration so the anesthetic could take full effect.

In a 150-patient breast augmentation series under tumescent anesthesia, the team allowed 45 minutes after infiltration for the local anesthetic effects to take place before surgery began. Operating time averaged 45 minutes, and recovery-room time about 125 minutes. Minor complications were reported in 5.3% of patients.
Epinephrine needs time to constrict blood vessels, and anesthetic needs time to settle into the tissue. Rushing is how awake surgery goes wrong.
