Awake Liposuction and J-Plasma: What the Evidence Shows
Liposuction under local anesthesia has the stronger track record. The skin-tightening claims made for J-Plasma are less established.
Liposuction under tumescent local anesthesia
Tumescent liposuction was designed to use dilute lidocaine and epinephrine, without general anesthesia. Published single-practice series report low complication rates, including a series of 4,380 patients and a later one of 9,002 patients. These are valuable, but observational, and describe the experience of experienced operators with carefully chosen patients.
The technique limits how much can be treated at once, because the lidocaine dose is capped by body weight. Review articles on liposuction describe how to calculate those limits.
What is J-Plasma / Renuvion?
Helium plasma radiofrequency (marketed as Renuvion, formerly J-Plasma) is an energy device used under the skin during or after liposuction to heat tissue in the hope of tightening skin.
What the evidence says
A 2025 systematic review reports that the device coagulates subcutaneous fat, and that the evidence does not support skin tightening beyond what may be achieved without it. We advise you to read the abstract in the references. Case reports describe serious complications such as subcutaneous and mediastinal emphysema after Renuvion treatment, including one involving combined VASER liposuction.
What this means for you
- We do not promise a percentage of tightening.
- Significant loose skin is usually better treated by excision surgery.
- Regulatory status and approved uses in Canada should be verified before treatment.
- Performing J-Plasma under local anesthesia does not remove its device-related risks.
